Forms are separate for GHMC and Non-GHMC areas. Below are separate forms. GHMC Forms are available in both English and Telugu for download. These same forms would be used on survey day. Household Survey Pre-Visits In Progress

The Telangana Government’s intensive household survey process has been initiated on field today with pre-visits. The Pre-visits would give away survey forms and answer questions people have on the survey and the process. The pre-visits are planned for today and tomorrow as a preparatory exercise for the survey. GHMC is makingt elaborate arrangements to take up the Comprehensive Household Survey on August 19. It appealed all the citizens to be at home on August 19 for giving particulars to the Enumerators. The main aim of the survey was to create a database of all the households, which will be used to help the poorer sections of the society with government schemes. Also, if a citizen expected to need an LPG gas connection, a passport or other imported documents from the government in the future, his household’s details would need to be in the database which would be created with this survey. HC Rejects To Stay Survey; TS Govt Assures Answering Survey Voluntary

The Telangana government on Thursday cleared the air that the survey on August 19th is voluntary and not mandatory. The survey is done for sprucing up welfare programs and plug leakages and fix irregularities, the Govt informed the court. Earlier, Justice Vilas Afzulpurkar of the Hyderabad High Court admitted the writ petitions field by Rajasekhar Tulasi and Seethalakshmi, called for records and posted the case for final disposal. R. Raghunandan Rao, senior counsel appearing for Seethalakshmi, contended that Parliament has already enacted the Collection of Statistics Act, 2008 and the Collection of Statistics Rules 2011.
